Senior Primary Care in St. George, Utah

Ongoing care for older adults that makes time for your goals, your medications, and the people who help you—at the clinic or, when travel is hard, at home.

Care Built Around Your Goals and Routines

Good care for older adults starts with what matters to you: the daily routines you want to keep, the activities you enjoy, and how independent you’d like to stay. Your provider uses those goals to guide decisions about treatment and prevention.

Senior primary care brings the pieces together—medications, ongoing conditions, preventive guidance, and advice from specialists or a recent hospital stay—so you have one practice helping you see the whole picture.

Need a sick visit or telehealth appointment? Those options are on our primary care services page.

How We Support Older Adults

Senior & Geriatric Care

Senior and geriatric care is comprehensive primary care shaped around the needs of older adults. Visits look at the whole person: current health, ongoing conditions, medications, and what you want to keep doing day to day.

Over time, your provider gets to know your history and can help you weigh choices, follow up after specialist or hospital care, and adjust plans as your needs change.

Visits may include

  • Reviewing all medications together
  • Checking in on ongoing health conditions
  • Preventive guidance suited to your age and health
  • Talking through results and advice from specialists

Senior Care at Home

For some older adults, getting to the clinic has become hard. Senior care at home is home-based medical care that can be discussed as an option when traveling is difficult.

Whether a home visit is possible depends on your situation, including your health needs, location, and coverage. Call to talk it through; our team will explain what may be arranged. Home visits are part of Peak’s broader mobile medical visits.

Can a family member or caregiver join the visit?

Yes. With the patient’s permission, a family member or caregiver is welcome to join and take part in the conversation. Many patients find it helpful to have someone there to share observations and keep track of next steps.

Can I discuss medications with the provider?

Yes. Reviewing medications is a regular part of senior primary care. Bring all of your medications, including over-the-counter products and supplements, or an up-to-date list so your provider can go through them with you.

Can Peak help with follow-up after a hospital stay?

Yes. Peak Medical offers hospital follow-up visits to review discharge instructions, medication changes, and next steps. Call the clinic after discharge to arrange a visit, and bring your discharge papers and medications.
